Your services will begin with your Care Coordinator conducting an initial assessment. Your Care Coordinator will maintain monthly phone contact with you and will be your connection to services.
At the end of your two years of service, your care coordinator will develop a transition plan with you that will include all the contact information of the providers with whom you have been working. We will connect you to civilian support systems, OIF/OEF resources and the best benefits coverage for your future.
