A statewide program of Denver Options that provides care coordination to children, adults and military personnel (OIF/OEF) with traumatic brain injury.

Our Care Coordination system draws from Denver Options’ 15 years of experience and excellence in service coordination. Colorado Connections’ Care Coordinators work with customers to find the best care and support systems to meet their individualized needs. We are dedicated to helping you:
